CONSULTATION REQUEST FORM

Please fill out the following information to sign up for ASRL Consulting Hours. Once the form is completely filled out and submitted, someone from ASRL will reach out to you and schedule your time. This form should only be filled out by faculty/staff members. If you are a student that wishes to have a consultation with ASRL, your faculty advisor must submit this form on your behalf.

First Consultation Free: First time consultations per this form are limited to a 30 minute time block and free of charge. If you require more than 30 minutes for the consultation request you submitted, we will negotiate a service contract to help you with your work. This fee can be paid via JV transfer between departments on campus.

Future Consultations: If you would like a consultation for a different project or topic in the future, the first consultation for each new project/topic form is also free. If you require more than 30 minutes for the consultation, we will negotiate a service contract to help you with your work. This fee can be paid via JV transfer between departments on campus.
