Welcome to ETSU Department of Family Medicine your Patient Centered Medical Home. We are a Level 1 Recognized 2008 Patient Centered Medical Home by the National Committee for Quality Assurance. The Patient Centered Medical Home is a health care setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient's family. Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner.

We here at ETSU Family Medicine want to ensure that you are the healthiest you can be. Your health care team will make every effort to teach and encourage you and your family to make better choices and reach healthier lifestyle.
