All students are required to purchase and maintain health insurance as a condition of enrollment at Quillen. This requirement must be satisfied by purchasing insurance and providing proof that you have coverage. You many contact
Fortegra, an AMA sanctioned company, for coverage, or any other alternate provider. All students must provide the Student Support Services Office evidence of health insurance by completing the Insurance Declaration Form on an annual basis. The form, along with the submission due date, will be emailed to all medical students. Failure to respond will result in the premium being automatically included on the Fall fee bill.
