Practice


Practice Network Clinics

  • Practice network clinics had 48,586 patient encounters in AY2016
  • Approximately $1.3 million in uncompensated care was delivered
  • Received $1 million from HRSA for the construction of the new Johnson City Day Center which serves the homeless
  • CHC Recognized as a National Quality Leader and a Health Center Leader plus PCMH recognition. This indicates that we are in the Top 30% of al health centers.
  • CHC accepted to the Health Center Medical-Legal Partnership Learning Network
  • Clinic patients are: 32% self-pay; 35% Medicaid; 12% Medicare; 18% insured; and 3% other
  • CHC accepted to participate in the Team-Based Care Learning Collaborative supported by CHC, Inc. and the Weitzman Institute
