| The mission of University Health Services is to provide acute and episodic care to university employees. University Health is a nurse managed clinic and the health care providers are Board Certified Nurse Practitioners. Patients may be referred to the precepting physician, Dr. Lubrano of ETSU Physicians, on a case to case basis. Referrals to local specialists and/or to local primary care physicians, if you have no medial home, will be made as deemed necessary by the health care provider. |

| Appointments are
same day appointments.Call 423-439-4225 early
in the morning to obtain your appointment slot. If an
appointment cannot be made that day, one will be scheduled
between 8:00 and 9:00 am the next day, if possible.
Appointments are not scheduled into the future. Bring a picture
ID, your insurance card, and all your medicines or a list of
your medication to your appointment.

| University Health Services accepts cash, credit card, & ID Bucs. Insurance may be filed for most insurance plans. You will be responsible for all charges that your insurance company does not cover. |
| Payment is expected at the time of clinic visit unless
prior arrangements have been made.If the patient is unable to
pay for the visit that day, a financial agreement will need to
be signed by the patient. Payment is due in 30 days. Faculty
and staff may have the option to have the payment deducted from
their paycheck.
