Visiting Student Application Information

Please Note:

• Applicant Status:  Visiting student applicants must be in the fourth year of medical school.
• Application Submission:  Completed applications MUST be submitted and received before availability of slots will be discussed.  ALL visiting student applications MUST be submitted 8 weeks prior to the desired rotation dates.
• International Medical Students:  Quillen accepts international applications only from international schools with which we have an exchange program.

Visiting Student Learning Opportunity (VSLO)

• Quillen College of Medicine at East Tennessee State University will only be using VSLO, the Visiting Student Learning Opportunity, to receive visiting student applications.  Applications may be submitted through VSLO tentatively in May each year.  To apply to our institution, please complete and send us a VSLO application for your preferred electives and dates.

 

MD and DO Students

• VSLO is only available for use by students enrolled in a U.S. LCME accredited school or a VSLO participating COCA accredited AACOM member college.

 

 

Visiting Student Required Documents

• Pre-Decision Documents

  o   Step 1 report required for MD students and COMLEX score report required for Osteopathic Applicants  

  o   Drug Screen Results (must be within six weeks of rotation) 

  o   Photograph 

  o   Personal Statement 

  o   CV/Resume 

  o   AAMC Standardized Immunization Form 

  o   Mask fit documentation 

  o   Criminal Background Check 

• Post- Decision Documents

  o   Date of Birth 

  o   Transcript 

  o Ballad Health Orientation Book

  1. Next to number 1. Select and review the Student Orientation Information Booklet
  2. Once you have reviewed the Orientation Information Booklet complete the Confirmation Form in Step 2
  3. For “Instructor’s Name” enter Kortni Dolinger
  4. Once you accept save the “Student Orientation Confirmation” page and submit to the assignment tab

  o   Ballad Health Confidentiality Agreement

  o   Acknowledgement of ETSU QCOM Policies 

  o   Personal Health Insurance 

  o   HIPAA Completion Certificate
