Title: Reducing Childhood Obesity and Chronic Disease in Central Appalachia – Data Analysis and Planning to Support Promising Practices and Strategies

Specific aims of project: (1) aggregate existing data to provide a picture of the current health condition of central Appalachia, in terms of childhood obesity, (2) inventory the current efforts to combat childhood obesity using survey methods and qualitative focus groups, (3) identify promising policies and practices that reduce the incidence of obesity and chronic disease, and (4) engage health professionals, community members, and grantmakers to identify successes, challenges, and strategies addressing childhood obesity in central Appalachia. - Source: Tennessee Department of Health

Title: Statewide Healthcare Provider Assessment of Diabetes Practices

Specific aims of project: Conduct an assessment of current provider practices for the prevention, treatment, and maintenance of diabetes in Tennessee. Based on results, develop provider training ; 1)To improve healthcare provider knowledge of prediabetes, diabetes and hypertension disease management and promote referrals to DPP LCPs, Diabetes Self-Management Programs blood pressure control and 2) decrease morbidity and mortality due to prediabetes, diabetes and hypertension by controlling blood glucose and blood pressure. - Source: Mountain States Health Alliance and Wellmont Health Systems
