SOCIAL SERVICES ASSOCIATE
Knowledge, Skills, and Abilities (KSAs): Knowledge of available community resources. Ability to interact with a wide range of individuals with varying degrees of social and/or medical issues.
Role Description
Responsible for providing essential support to vulnerable populations by facilitating access to critical services. Coordinates appointment scheduling to ensure timely medical and social service access, organizes transportation to appointments for individuals in need, and identifies and procures social support resources to enhance overall well-being. Offers interpretation services to bridge communication barriers, ensuring clients understand their rights and options. Acts as an advocate and serves as a liaison between clients and service providers, promoting equitable access to resources and empowering individuals within marginalized communities.
Typical Functions:
Assists clients in scheduling medical, mental health, and social service appointments.
Follows up to confirm appointments and remind clients of upcoming visits.
Offers interpretation assistance during appointments and meetings to ensure effective
communication.
Translates important documents and materials as needed.
