SOCIAL SERVICES ASSOCIATE

Role Description

Responsible for providing essential support to vulnerable populations by facilitating access to critical services. Coordinates appointment scheduling to ensure timely medical and social service access, organizes transportation to appointments for individuals in need, and identifies and procures social support resources to enhance overall well-being. Offers interpretation services to bridge communication barriers, ensuring clients understand their rights and options. Acts as an advocate and serves as a liaison between clients and service providers, promoting equitable access to resources and empowering individuals within marginalized communities.

Typical Functions:

Assists clients in scheduling medical, mental health, and social service appointments.
Follows up to confirm appointments and remind clients of upcoming visits.
Offers interpretation assistance during appointments and meetings to ensure effective communication.
Translates important documents and materials as needed.

Levels

Social Services Associate 1 Entry-level position responsible for providing essential support to clients by coordinating transportation, scheduling appointments, and offering reminders. This role requires excellent organizational skills, strong communication abilities, and a compassionate approach to working with individuals from diverse backgrounds. Education/Experience: Typically requires a high school diploma or equivelant.

Social Services Associate 2 Responsible for coordinating with community partners, facilitating patient/client screenings, and supporting effective communication between providers and patients. This role requires strong organizational skills and a deep understanding of community resources and social services practices. Education/Experience: Typically requires a high school diploma and one to three years of relevant experience, or an equivalent combination of education, training, and experience. Applicable certification may be required.

Social Services Associate 3 Advocates for patients and clients, connects them with specialized services, and supports them in navigating the complexities of medical, legal, and social systems. This role requires a deep understanding of medical benefits, strong advocacy skills, and the ability to establish educational and support programs for individuals and groups. Education/Experience: Typically requires a high school diploma and three to five years of relevant experience, or an equivalent combination of education, training, and experience. Applicable certification may be required.
