ETSU Employee Forms for Educational Benefits

ETSU Employee Fee Waivers, Tuition Reimbursements & Spouse/Dependent Discounts will need to be submitted via the electronic benefits portal. For more information visit the Human Resources website or call (423) 439-4457.

ETSU Spouse/Dependent Fee Discount	50% off in-state maintenance & program service fees; Part-time is pro-rated	Undergraduate only	Spouse or dependent child 26 years old or younger as of 1st day of class	ETSU Employees may use this for spouse/dependents at ETSU & Other TBR/UT Schools
Request for Tuition Reimbursement	up to 6 credit hrs successfully completed	Graduate or Undergraduate	Faculty and Staff employed at least 6 months	Full-time employees are reimbursed at ETSU rate, part-time employees are reimbursed at a pro-rated percentage
