Deadlines and Forms:
Several forms must be completed at designated times during your program of study.
All of them can be accessed on the Graduate School page, “Forms.”
Some of these forms will be initiated by you, whereas others will be initiated by your thesis chair if you are writing a thesis. Please keep your chair informed as to which form they need to initiate and when, as they may be juggling several thesis students.
While a general guideline for when to submit these forms is provided below, please note that the academic calendar lists the hard deadlines by which the graduate school must receive these forms. Note that the graduate school has helpfully color-coded the dates that pertain to graduate students in green.

Apply to Graduate |
Completed at the beginning of the semester, before you plan to graduate (for a fall
graduate, Sepetember 1st). |
Committee Appointment | After your thesis prospectus has been approved by your thesis chair. |
Manuscript Review (three signatures) | After you have completed your thesis defense and your chair has accepted your final revised thesis. |

Results of Culminating Experience | When you have passed your comprehensive exams, the coordinator of graduate studies will complete this form. |
Notification of Thesis Defense | At least ten days prior to your thesis defense (though earlier is better), your chair may fill out this form when you have a date and time scheduled for your defense. |
Results of Thesis Defense | Your chair may complete this form when you have satisfactorily passed your thesis defense. |
